A 42-year-old female patient was referred to our clinic at the School of Dentistry of the University of São Paulo in November 2004, presenting a deficient restoration in the upper left central incisor. The clinical examination revealed no gingival retraction or any signs of gingival inflammation and, therefore, previous periodontal treatment was not considered. The patient presented a high lip line at full smile and a thin tissue biotype. This combination characterized a high-risk situation from an anatomic point of view, which required careful preoperative planning and cautious surgical execution.
